MEDICAL HOME
Pregnancy Medical Home model is an enhanced prenatal care initiative designed for women enrolled in Medicaid or at risk for maternal morbidity. By addressing health inequities and providing comprehensive support during pregnancy, delivery, and the postpartum period, this model works to:

Reduce the overall cost of medical care during pregnancy and infancy

Empower mothers through education, resources, and personalized care
How Does it Work?
Maternal Health Specialists (MHS) are the heart of the PMH program. These professionals are embedded within prenatal practices to offer tailored care and support. Here's what they do:

Risk Screening: Assess pregnancy-related health conditions, mental health issues, substance use, social determinants of health, and domestic violence.

Enhanced Prenatal Care: Provide individualized education, health promotion, and connections to community resources.

Direct Support: Offer one-on-one guidance to coordinate access to healthcare services and medical specialties.

Maternal Health Education: Share vital information on interconception care, safe sleep practices, and medical self-management.
The PMH program represents a significant step forward in ensuring that every expectant mother receives the care, support and resources she needs.
If a participant screens positive for risk factors, the
MHS takes immediate action to initiate enhanced
prenatal services, coordinate care, and connect them
to the necessary resources to ensure the best
possible outcomes.

PMH is locally operated through REACHUP, Inc. in collaboration with Healthy Start Coalition of Hillsborough County, USF Health, Tampa Family Health Centers. Funding is generously provided by the Healthy Start MomCare Network, Inc., and the State of Florida, Department of Health, and State of Florida, Agency for Health Care Administration.
