How do I sign-up for MomCare?

MomCare Application Process (easy to follow instructional flyer)

MomCare’s health insurance is available to women who have proof of a positive pregnancy test on a physicians or pregnancy centers letterhead that states expected delivery date.

You must also have a maximum household income at or below 185% of the poverty level and be a United States citizen or a legal alien and a Florida resident. See income chart.

Applications are available at physician’s offices, the Department of Health, the Department of Children and Families, and other qualifying pregnancy testing centers.
